Internship Instructions

Submission Materials

The following materials should be submitted to complete the application:

1) A curriculum vita of relevant work, education, and clinical experiences.

2) An official transcript of all post-baccalaureate credits.

3) A completed APPIC application (both parts I and II). This form can be download by clicking here -> Download APPIC Application.

4) Three letters of recommendation from people who are familiar with your clinical work.

Your completed application must be received by November 3rd to be considered for the following internship year. Do not fax your application.
